Termination of Enrollment

Your enrollment will end on the last day of the pay period in which:

You are separated from your job, unless you are separated under circumstances that allow you to continue your enrollment, see section titled "Circumstances Permitting Continuation of Enrollment"

• If you become ineligible for coverage because of a change in your employment status.

• You die, and there is no eligible survivor annuitant to continue the enrollment.

Your enrollment also will end on:

• The last day of the pay period that includes your 365th day of continuous nonpay status.

• The day you are separated, furloughed or placed on leave of absence to enter miltary service for more than 30 days, if you elect to have your enrollment terminated, see section titled "Circumstances Permitting Continuation of Enrollment".
