- If you can you answer "YES" to the 4 questions on the perforated attachment to the form
DSP-82 (red application) you may renew your passport.

- Complete and sign your application and enclose your Expiring/Expired Passport (you
must have this in order to use form DSP-82). Your passport will be valid for 10 years.

- Enclose 2 identical passport photos...hint- when you go to get your photos, make sure
you tell the photographer that you need "U.S. Passport photos- every country has
different photographic requirements.

- Enclose a check or money order payable to PASSPORT SERVICES for the amount of
$55.00
|
- Attach your letter to the Passport Agency specifying your delivery instructions.
|
- Place all of the above documents into a franked envelope and address it to: Passport
Agency, 1111 19th Street, NW, Washington, DC 20515
