| First Time Applicants: |
- Complete the application DSP-11 and submit a certified copy of your birth certificate. Do
not sign this application. The signature must be witnessed by a Passport Agent
(available in B225 Longworth on a walk-in basis).

- Enclose 2 identical passport photos...hint- when you go to get your photos, make sure
you tell the photographer that you need "U.S. Passport photos- every country has
different photographic requirements.

- Enclose a check or money order payable to PASSPORT SERVICES for the amount of
$65.00

- Attach your letter to the Passport Agency specifying your delivery instructions.
|
- Place all of the above documents into a franked envelope and address it to: Passport
Agency, 1111 19th Street, NW, Washington, DC 20515
