Reporting Your Passport Lost or Stolen

• If your passport has been lost or stolen, you must complete form DSP-84.

• If your are applying for a new passport and cannot find your expired/expiring passport, your must complete form DSP-84 and submit along with your form DSP-11.

• Place all of the above documents into a franked envelope and address it to: Passport Agency, 1111 19th Street, NW, Washington, DC 20515
