- Children from birth to 18 years of age must complete form DSP-11 (brown form)

- Passports for children are valid for 5 years. Each time the passport is renewed the DSP-11
must be completed.

- If you are renewing a child's passport, complete the application and submit the
expired(ing) passport. Do not sign this application. The signature must be witnessed by a
Passport Agent (available in B225 Longworth). If your child is able to make his/her own
signature, we can witness it. If the child cannot sign for him/herself, we can witness your
signature for the child.

- Enclose 2 identical passport photos...hint- when you go to get your photos, make sure
you tell the photographer that you need "U.S. Passport photos- every country has
different photographic requirements.

- Enclose a check or money order payable to PASSPORT SERVICES for the amount of
$40.00
|
- Attach your letter to the Passport Agency specifying your delivery instructions.
|
- Place all of the above documents into a franked envelope and address it to: Passport
