Amending Your Passport


You can amend your passport in several ways. If you changed your name since your passport was originally issued (such as legal name change, name change due to marriage, adoption, or divorce) you must complete form DSP-19. If you have run out of pages for your passport, you can have additional pages added by completing a form DSP-19. If your passport was issued with incorrect information or if your name was misspelled you should complete form DSP-19.
Complete and sign form DSP-19 for all amendments to your passport
Enclose your passport for amending.
There is no fee for this service.
Attach your letter to the Passport Agency specifying your delivery instructions.
Place all of the above documents into a franked envelope and address it to: Passport Agency, 1111 19th Street, NW, Washington, DC 20515
