You can amend your passport in several ways. If you changed your name since your passport
was originally issued (such as legal name change, name change due to marriage, adoption, or
divorce) you must complete form DSP-19. If you have run out of pages for your passport, you
can have additional pages added by completing a form DSP-19. If your passport was issued
with incorrect information or if your name was misspelled you should complete form DSP-19.
