House Employees must:
- Complete this form when you need to change your name or address or if you no longer want your paycheck electronically deposited.
- Complete the Oath of Office Form, Sections A and B.
- Do not forget to sign it under Section C.
- Only complete the right hand side of Section B, if you want your paycheck to be mailed to a new address.
- Do not complete this side if you still want your paycheck to be electronically deposited.
- Remember that all forms which affect the current month's pay must be received in the Office Finance by the fifteenth of the current month.

Members must:
- In lieu of completing a form, you just have to contact a Members' Services Counselor when you need to change your name, address, or phone
number.
- Remember that all requests for changes which affect the current month's pay must be received in the Members' Services' Office by the fifteenth of the
current month.
